Through the contact / eligibility form, we collect information you submit, such as name, phonetic reading, age, email address, phone number, answers about diagnosis and treatment history, medication status, consent confirmations, and free-text messages.

Among these, details about diagnosis, treatment history, and medication status constitute “sensitive personal information” under Japanese law. We collect them only with your consent at the time of form submission, and use them solely for eligibility assessment and contacting you.
