Information from the form is used only to respond to your inquiry, as a reference for eligibility assessment, and to contact you when needed. Information from visit analytics is used to understand and improve how the Site is used and operated.

• Contact form delivery: email delivery service Resend (used to send your submission to the Clinic’s designated address)

• Site hosting and (when you consent) visit analytics: Vercel, Inc. / Google LLC (Google Analytics)

Some of these processors are located in the United States, and form contents may be transferred there. By submitting the form, you consent to this transfer.

Aside from the above, except as required by law, we do not disclose or sell personal information to third parties without your consent.

5. Visit analytics and consent

The Site uses Google Analytics (Google LLC) and Vercel Analytics (Vercel, Inc.) for visit analytics only if you consent. If you decline, analytics are not enabled.
